I understand that while every care will be taken by the leaders in charge to ensure the safety and comfort of my child/ren at Life Anglican Church, its leaders and the Life Anglican Church are not in any way responsible for any accident or sickness that may occur or happen through any circumstances.

I authorize you in the event of any accident or sickness, to obtain ALL necessary medical assistance and hospital accommodation and in this event, I agree to pay all such doctors, hospital fees, and any expenses incurred on behalf of my child.

In the event of any emergency necessitating a surgical operation, I authorize such operation and the use of an anesthetic providing it is carried out by a qualified medical practitioner.
